Health Exam for French Visa

My host family says that they have to turn in a copy of a health exam with my contract to get it approved by the DIRECCTE but when I went to the clinic here to get the exam they said that usually whoever is requesting the exam would send a form detailing what questions need to be answered and what illnesses need to be checked for... From where am I supposed to get this form?? The French Consulate's website told me I wasn't going to need an exam until I arrived in Paris and now I need to get an exam like... yesterday (apparently) and I don't have whatever form I'm supposed to bring for the exam... !!! Any help?
